Summary of Responsibilities:

In conjunction with the liaison staff, obtains referrals, verifies insurance and coordinates all aspects of the admission process for the unit. Engages in marketing activities within the referral community.

This is a 40 hour per week position working 9:00am to 5:30pm.


  • Associates Degree in Human Services/Nursing or related field preferred.

  • LPN/RN preferred.

  • Long-term care experience preferred

  • Familiarity with Attorney General's Long-Term Care Regulations preferred.


  1. Possesses a thorough understanding of the scope of TCU services and is able to independently review and accept/decline admissions in a timely manner.

  2. Collaborates with the interdisciplinary team as needed for potentially complex admissions.

  3. Communicates regularly with the referral network and follows up on all referrals in a timely manner.

  4. Communicates with discharge planners, agencies and families regarding admission decisions and arrangements for admissions.

  5. Collaborates with LTACH liaison staff to identify TCU level and site neutral referrals.

  6. Schedules incoming patients, and works with referring entity and interdisciplinary team to order any needed patient specific equipment and ensure arrangements for specialty services such as dialysis

  7. Verifies patient insurance prior to admission, ensuring primary and secondary payment sources.

  8. Completes PASSRs and other pre-admission evaluations as required by regulations and facility policies

  9. Answers telephone inquiries regarding facility's programs and availability of beds.

  10. Conducts tours of the unit for potential patients, families, community, and representatives from placement agencies and organizations.

  11. Identifies potential referral sources within referral area and initiates/participates in marketing activities

  12. Assumes responsibility for review and completion of admission documents with patient and/or legal representative, including but not limited to admission agreement, assignment of benefits, choice of attending physician, medical consents, Patient Rights, facility and patient responsibilities.

  13. Maintains professional confidentiality.

  14. Maintains log of all referrals and other admission records as required by regulation and facility policies.

  15. Maintains current knowledge of federal and state regulations as they relate to Admissions Department. Attends and participates in in-service educational programs, particularly those that relate to the psychosocial needs of the elderly, community resources, and admission requirements (Title XVIII and Title XIX), etc.

  16. Maintains a flexible work schedule so as to be available to work evenings and weekends as required.

  17. Attends outside meetings as required.

  18. Organizes and compiles admission packets.

  19. Prepares monthly statistical data reports of admissions, CQI data and presents results at quarterly meetings.

  20. Performs other related duties as assigned.
