Case Manager, Per Diem
SUMMARY OF RESPONSIBILITIES: In collaboration with the treatment team and Quality Management, the Case Manager ensures that, from admissions to discharge, a patient receives appropriate, timely and high quality care in the most suitable setting while addressing an individual's needs to achieve quality cost-effective outcomes. This includes utilization review/cost containment and discharge planning responsibilities.
1.Graduate of an accredited School of Nursing.
2.Five years integrated clinical nursing experience.
3.One to two years of related clinical, continuing care, UR or case management experience preferred.
4.Current Massachusetts nursing license.
5.Bachelor's degree preferred.
6.Case Management experience preferred.
7.Certified case manager or completion of a certificate program preferred.
8.Excellent written and oral communication skills required.
9.Effective interpersonal skills required.
10. Excellent computer skills required included MS Office and Meditech, Midas highly desired.
