Job Description

Medical Record Coder is responsible for timely review of patient records in order to identify an appropriate selection of codes, which will accurately reflect the reason for admission, extent of care received, and level of severity of illness. Coder is further responsible for insuring that all data elements required for federal and state reporting are collected and included in the patient’s demographic record.

Qualifications

POSITION GOAL:
Medical Record Coder is responsible for timely review of patient records in order to identify an appropriate selection of codes, which will accurately reflect the reason for admission, extent of care received, and level of severity of illness. Coder is further responsible for insuring that all data elements required for federal and state reporting are collected and included in the patient’s demographic record.

1. POSITION REQUIREMENTS

A. EDUCATION:
Graduation from an accredited HIM program, OR CCS (CCA) certification. Consideration given based on employee entry coding exam, able to acquire minimal CCA from AHIMA program with progression to higher level CCS certification within a 3-5 year time frame

B. EXPERIENCE:
Minimum of two years ICD-9-CM and CPT-4 coding experience in acute care facility or strong coding exam performance. Proficient in ICD-10 coding. Requires strong verbal skills in interpersonal situations.

2. CAREER PATH

Advancement into positions in the department that may require technical, or advanced educational training.

3. SPECIAL CONSIDERATIONS

Age legend is not applicable. This position has no responsibility for the assessment, treatment, or care of patients. However, the ability to communicate and interact with patients of all ages is required.
