Prevention Administrator - SHCN
This position is responsible for research, development and improvement of prevention programs and strategies throughout the Steward Health Choice Arizona (SHCA) network. Responsibilities include: developing collaborative partnerships with community and provider agencies, clinicians and community members to locate resources and enact prevention efforts and initiatives that are effective and sustainable, providing training and technical assistance to staff and prevention providers; monitoring prevention provider contracts and budgets; researching and writing grants for possible funding; preparing reports to be distributed to various agencies throughout the SHCA network and state; and monitoring prevention providers for compliance with SHCA, state and federal regulations.
- Conducts annual needs and capacity assessments. Develops and monitors prevention program contracts and budgets; monitors and evaluates prevention programs and outcomes, conducts site visits; develops and communicates strategies to improve prevention efforts; and makes recommendations in accordance with findings.
- Analyzes data and information for special projects; writes project and progress reports to SHCA, its stakeholders and AHCCCS, as required. Reports all prevention activities to AHCCCS through the System of Care plan, quarterly updates and annual reports.
- Provides prevention training, specific case consultation, performance improvement and other technical assistance to providers. Collaborates with community groups, and performs monitoring and resolution efforts related to prevention programs.
- Assists in the development and maintenance of project standards, policies and procedures, protocols and gap analysis.
- Coordinates prevention related events and/or methods of outreach to underserved populations through community forums, focus groups, and/or by participating in health fairs, as appropriate. Actively participates in HCIC committees, including but not limited to the Northern Arizona Children’s Council, the Adult/Child Committee, and Continuous Innovation Committee.
- Researches grant opportunities for prevention programs; writes grants or assists in the development of grants for prevention programs, as appropriate.
- Bachelor’s degree in the behavioral health or related field or
- An equivalent combination of education and experience sufficient to successfully perform the essential duties for this position.
