Summary:

Responsible to Trumbull Regional Medical Center for supporting the corporate mission, vision, and values. Reporting to the Chief Quality Officer and working in partnership with the hospital administrative team to investigate, resolve, document and report patient and visitor compliments and concerns. Facilitate resolution of complaints and grievances of patients, family members and visitors. Develop, implement and participate in customer service and patient relations initiatives. Provide monthly accountability and variance analysis of customer service outcomes.

Minimum Qualifications:

• Bachelor’s degree in social work required.
• Current Ohio license as a Licensed Social Worker by Counselor Social Work Board required.

Job Responsibilities, including, but not limited to:

• Responsible to Clinical Integration Group for providing professional counseling services directed towards assisting patients and families meet their psychosocial and discharge planning needs.
• Responsible to the customer for courteous, accurate, timely service.
• Collaborates with Care Coordinator to provide continuity of services, appropriately assesses patient needs, including psychosocial, financial, post-acute, supportive, potential abuse, spiritual, and ethnic needs if applicable, develops plan and coordinates post-hospital needs in a timely manner, maintains social service records and related documentation, demonstrates knowledge of Performance Improvement, serves as educational resource and maintains current knowledge, and performs other duties as assigned.
